Add support for node18

Keeping this repo current, test it against node18 and make changes. If backward-compatibility cannot be maintained with other versions, make it a major.
- Add node 18
- Add a section in the README that describes the node versions that it's been tested against. Some of them may fall out and be unsupported as upgrades continue down the road.
