[Request] Azure Data Studio Extension

Create an Azure Data Studio extension for DBA Multitool.

Describe the solution you'd like
It may:

• Link to documentation
• Link to files to install
• Have shortcuts to run each command in a new connection tab
• Generate a notebook with the results of sp_doc as markdown
• Visualize all/some of the results for sp_sizeoptimiser

Additional context
Some blockers exist:

• microsoft/azuredatastudio#14715
• #197
  • ADS has the same line feed issue as SSMS
  • A workaround for both could be replacing all CHAR(10) + CHAR(13) uses with explicit empty lines inserted into the results
• microsoft/azuredatastudio#2768
