The Monarch Initiative aims at integrating many different ontologies, so to organize biomedical knowledge and enable discovery & precision medicine.

They have a text annotator which already matches many terms in a free text to varied biological entities: https://monarchinitiative.org/tools/text-annotate

I would imagine that the code is available somewhere, even though I would not know exactly where.
