Dynamic user-configurable custom fields

Currently the set of custom fields is fixed with:
- Epic Name
- Epic Ref
- Story Points
Acceptance Criteria(closed in #52)
Extend this with "dynamic" custom fields.
The user should be able to configure in a simple config file:
- the Jira field name
- the customfield ID
- the Issue.field_name to be used in the app

Removing from milestone 0.2.4 as I don't think this needs to be solved just yet
