Activate dynamic form for flavors

We may want the dynamic form to be active on individual flavors. This requires a minimum dynamic form setup in the config.
We've activated the dynamic form on the following flavors: Lake Washington, Waterfront, Willamette, gtopenspace, Snoqualmie, Trees, and Raingardens.
Not sure what to do with trees--its form has new field types (geocoding) not currently supported by the dynamic form. We could add them though...
