Have 'skip to main' cycle between multiple main regions

Given that if there are multiple main regions specified, it's not really possible for us to know which is the most important, cycling between them seems like a reasonable approach (that's what JAWS does).
This was raised first in #370 (thanks for the input @carmacleod).
