Add example with attachment for CareCommunication

Add an example concerning, how an attachment would be included in a CareCommunication message.

@MedComAnders - in the commit, the example is included. It is possible to build the example/IG (see fsh-generated -> resource folder), however the attachment is not included when viewing the example/IG online. Will you help fixing it?
