Missing MIT License and copyright/attribution

I'm humbled by the derivative work, but given the fact my code was copied verbatim in many places, my copyright and a copy of the MIT license needs to be included in the project:

The "acknowledgements" doesn't cover that.

@voutilad, we've made changes under #17, before merging, are the changes appropriate? Is there any other suggestion from your side? Feel free to just comment on the PR directly.
