Add GitHub Enterprise Server mention support and fallback when neither GitHub nor Azure Repos

Either figure out how to identify GitHub Enterprise Server, or switch to identify Azure Repos for "#" and otherwise "AB#". However, there could be a repo not GitHub nor Azure Repos.. in that case, should also figure out how to hide the mention command.
