USB/CDC no longer works with Mac OS X El Capitan/Sierra

The USB device presented by Cosa is not recognized by OS X El Capitan (10.11). From what I've read, the problem exists in Arduino as well (which also doesn't work on OS X Sierra (10.12)).
With my AdaFruit atmega32u4 with Caterina bootloader the serial port appears after a manual reset and programming works. As soon as Cosa gets control the serial device does not appear again.
I am investigating but at the moment have no idea what the issue is.

My mistake. Incorrect clock speed can do this!
