Remove "dead" condition automatically when healing from 0 to >0 HP

When a combatant transitions from 0 HP to 1 or more HP, the "dead" condition should be removed automatically.

Actually never mind--the "dead" condition should be permanent, and is only being applied for massive damage/insta-death. I think we don't need to do this at all.
