Hide combatants from player view

Once we build #51 it would be beneficial to be able to hide combatants from the player view. This would allow them to be loaded and have an initiative value but be undisclosed to players until the DM is ready for them to act in a way the players can perceive.
For example, an Invisible Stalker probably shouldn't show up in the player view before the players have figured out that it's there! Likewise, guards that are "off screen" could be teed up and ready to go but not revealed until the DM is ready for them to join the fray.
It's possibly a good idea for all loaded monsters to default to being hidden, requiring that they be explicitly made visible before showing up in the player view.
Done! Decided to make monsters default to concealed in the player view; they must be explicitly revealed using the new 'reveal' command.
