Change of Officer form/process

Need to have a form that allows users to select a position to "apply" for, and sends the request to a defined list of officers (based on the selection).
Notify the officers and have a mechanism for them to approve or reject the request.
Send notifications to the requesting user and a defined list of officers upon approval/rejection.
