Modified versions of the dark Flat UI theme by Kaiyuan Liu.
The theme is available on the VS Code Marketplace
From command palette, run:
ext install muhammadsammy.flat-ui-dark
Or search extentions within VS Code: click "view" in the menubar then "Extentions" and search for "flat ui dark".
The font used in the screenshot is Hack.
