map display for submissions

checklists, surveys and critical incidents should (optionally and off by default) support the display of the submissions in a map display as an extra tab.

What gets displayed on the map will respect filter parameters.

So far this seems good, and don't have additional feedback other than #601 . Will open other issues if anything comes up
