Transparency controls: Require “Reason: __________” comment by admin when using impersonate

Splitting up the sub-requests per @j-ed suggestion.
Enhancement: Require admins to enter a short reason why they are the impersonating the user. This reason would be able to be called back for things like activity log, notification (to users and others interested) and email alert to user.
