Clean-up use of exceptions

Because of the history of the code with several authors the use of exceptions (custom or standard) and how they are handled is quite different in different areas of the code. This should be cleaned up.

A partial cleanup was done as part of #134 and some more in #128.

A final cleanup can only happen in 0.6.0 series after removing the old "wrapper" CLI (from 0.5.x and earlier).
