Bug: Remove Button Not Functioning in "Add Insurance" Section

Describe the bug
Remove button functionality is not working in Add Insurance

To Reproduce
In Add Insurance Details user should be able to Remove the field by clicking on the remove button

  1. In side bar click on Patient
  2. Click on Add Add Patient Details
  3. Scroll down click on Add Insurance Details
  4. click on delete

Expected behavior
The user should be able to remove the insurance details field by clicking the "Remove" button, and the corresponding input fields should disappear from the form. The removal should not affect other data entered in the form
