Seeing questions that wonder if they are undefined and show up in all filters

I noticed a user that has a small number of Disagree questions, and one of them is misbehaving. On a fresh page load it shows up as it should, but dashed and wondering "Is this a undefined question?". It seems to appear on every filter and does not contribute to their counts.

I'm not 100% sure of all the details here. They change once I start categorizing it.

My suspicion is that the question somehow might not be in the master question list. It's also possible the cleanup branch I have in progress is contributing.
