Reintroduce Share/Print for calculator

The Share and Print widgets were disabled for the Survey calculator when it was moved from the individual profile pages to its own Calculator page (8767dd3).

Most of the technical functionality is still in place for creating sharable URLs, so this simply needs hooking up in the template again.
Although there is a print style sheet, the page elements changed during the move so this no longer works. The stylesheet will need to be amended and a print button hooked up on the template.
UI
We would need to find a place on the page to re-introduce the Share/Print buttons.
