PermitTypeMapped - additional permit type

Many jurisdictions issue combination permits which include multiple trades. We should consider adding a type of:
- Combination

A few other permits types that don't quite fit in the designated PermitTypeMapped:
- Siding (could go under Building or Roof)
- Signs
- Fences
- Elevators
- Solar Thermal (not PV)
- Wind Turbines
