For ease of review, I’m listing the synonyms I used for each condition below. In general I excluded codes that sounded like they were tests (ie measurements of antibody levels) and family history codes, but included conditions that were secondary causes of a condition we were interested in (ie myositis in sarcoidosis). I included one code in newborn (Scleroderma in newborn). I don’t think this will appear in the sample, but following the rationale of including history of disease. I also excluded the following conditions I was unsure about:
- (Ichthyosiform erythroderma) or (Sjogren-Larsson syndrome)(PH12.)
- Marinesco-Sjogren syndrome) or (disorder of metabolism NOS)(C37z.)
- Marinesco-Sjogren syndrome(X002H)
- Sjogren-Larsson's syndrome(X50IH)
(I needed to select them either way to save the list).

To remind myself of background, trying to capture all systemic inflammatory of autoimmune disorders which might indicate vaccination (directly or through immunosuppressing treatment) and be associated with Bell's palsy, transverse myelitis or GBS could become a very broad and hard to define list. We decided to approach this by including specified conditions identified in literature as risk factors for at least one of the study outcomes.
The protocol says we're looking for: Systemic inflammatory or autoimmune disorders combining: ankylosing spondylitis, antiphospholipid syndrome, Behçet disease, mixed connective tissue disease, rheumatoid arthritis, sarcoidosis, scleroderma, Sjögren syndrome, systemic lupus erythematosus, and sarcoidosis.
I note sarcoidosis is in the list twice - this should perhaps be amyloidosis and sarcoidosis. The protocol rationale was:
- Bell's palsy: Systemic inflammatory or autoimmune conditions such as amyloidosis, sarcoidosis or Sjögren syndrome are not themselves indications for vaccination, but would typically result in vaccine eligibility due to immunosuppressing medications, and will therefore also be combined for consideration as possible confounders.
- Transverse myelitis: Systemic inflammatory or autoimmune disorders associated with transverse myelitis include ankylosing spondylitis, antiphospholipid syndrome, Behçet disease, mixed connective tissue disease, rheumatoid arthritis, sarcoidosis, scleroderma, Sjögren syndrome, and systemic lupus erythematosus (Law et al. 2020 SPEAC case companion for transverse myelitis) These, or their treatment with immunosuppressing medication, were indications for vaccination in the ‘at-risk’ group and will be combined to explore for confounding.
- GBS: Systemic lupus erythematosus is also a risk factor for GBS, and its treatment with immunosuppressing medications is an indication for vaccination, and so this will also be considered a potential confounder (Pluta et al. 2011 JAMA).
We have codelists for rheumatoid arthritis, antiphospholipid syndrome, and SLE, so we're looking for ankylosing spondylitis, Behçet disease, mixed connective tissue disease, sarcoidosis, scleroderma, Sjögren syndrome. (?and amyloidosis).
On the synonyms,
Ankylosing spondylitis:
- agree with not including axial spondylitis/ non-radiographic spondylitis, as patients may not progress to AS, and also looking for the specific diagnosis, not looking more broadly for spondylitis associated with other conditions.
- This paper by Khan et al (2019) included a footnote on AS eponyms and synonyms:

Behcet disease
- NORD also has Adamantiades-Behcet's syndrome, would be covered by Behcet, but could be Adamantiades's syndrome
- search for synonyms found also known as' for malignant aphthosis and oculo-orogenital symdrome, but the latter could be vitamin B deficiencies and the former seems non-specific and rarely used for Behcet's and potentially not specific
