Fixes to the PNC medical history page

Steps to replicate:
- Fill out a pregnancy outcome form (I added 3 children), make sure delivery date is at least 3 days ago so the first PNC home visit becomes due
- Fill out the PNC home visit, record no family planning method selected
- Submit the form and then go to the PNC medical history page.
