apaxy is missing a favicon

apaxy is missing a favicon that should be included by default into apaxy.

Maybe @chrisssycollins could create one in pull request #103.

Error pages have been added in commit 0e69f5d but the favicon references in html has not been added.

It could be a good idea to create a favicon from the official logo made by @AdamWhitcroft.
