mesh/cols horrible example in documentation

I spent a couple of hours figuring this out, and it was not that pleasant…
From the documentation:
We have a 3 × 3 matrix with a table having 3 items per row.
Now, mesh/cols=3
could be any of the 3 3s.
Yes, I read the text, but it's hard to understand.
A better example would use 3 items per row for a 4 × 5 matrix.
In this way one would actually be able to understand what mesh/cols
should actually represent.

Feel free to provide a full better/improved example either here or directly as PR and we'll be happy to incorporate that.

Cool, I'll do that. I was kinda asking permission / relevance with this issue.
