I found two grammatical changes that need to be made on the textbook, both in Chapter 6. On the second paragraph of the opening of Chapter 6, you can delete the words "that was had" in the opening sentence: "The investigation found that many aspects of the NASA decision making process were flawed, and focused in particular on a meeting that was had between NASA staff and engineers from Morton Thiokol, a contractor who had built the solid rocket boosters." In that sentence, you can also delete the word "had" in "who had built the solid rocket boosters."
