Unexpected error when handling authentication request to identity provider.

User gets error after entering TOTP token:

We ran some tests and found out why this error occurs. The problem is that users press the login button multiple times (or press the Enter button on the keyboard and then immediately press the Sign In button.
To fix, we added onsubmit="login.disabled = true; return true;" to the template privacyIDEA.ftl in kc-otp-login-form #98
