[FQL] extend with support for adding/editing relations (and xlink:href)

We are currently missing an interface to the xlink:href attributes, the format attribute.
Also investigate whether extra syntactic sugar is needed to add xref element under a relation.
Required for proycon/flat#84

- Add FQL documentation
