Umbraco version 7.12.0 breaks UI

Due to an update to the link picker, the UI for the link picker is currently broken.

Here's an example of how the UI looks.

The layout issues appear to be caused by the manual addition of the querystring field as there is now a version in the core. Removing the code shown below fixed the UI problem. There are some other save issues which i think are caused by the PR itself.
