Ease of use: redeploy should create a new MCA entry instead of failing

Currently, if a redeploy is performed on a rholang file that does not already have a MasterDictionary entry, a failure is reported.
This is problematic for developing new rholang that belongs in the MasterDictionary, as doing a redeploy is the only way to register new/updated rholang code into MasterDictionary.
