i have observed this circumstance too. however, i think this is because while facing long document some part of its context will lose, which is something i can't fix. but i will try :)

This happend to me once I used the wrong model instructions.
For example, I Used:
v3.5-20241113.md
for the wrong Sonnet model.

i have updated the instructions to v4 (with v4-Lite), you could try them out!
