add missing documentation

Currently most of the code is covered with Doxygen documentation, but EXTRACT_ALL is enabled and a few bits are still left undocumented.

Disable EXTRACT_ALL and fix everything Doxygen complains about. Also look into used macros: expand those that would make the documentation easier to read, e.g. when using shortcuts for groups of similar functions.
