Add detection of migrated extensions

After migrating extension to new vendor, we get commits like this: flarum-lang/polish@3aeb851. This could be problematic, because translations exported to language packs are not automatically removed - such change needs to manually removed to avoid conflicts with new extension, which contains correct translation (flarum-lang/polish@8a44365).
