Anonymised dataset application form link broken

Good afternoon

I'm wondering how one requests access to the anonymised dataset for cleanEHR? The link to the form on ropensci is broken.

Thank Ed, it doesn't seem to me that it's fixed though.
Maybe we need to update the link in README?

Oh sorry - I just meant we have dealt with this data request. This is already open as a ticket and we'll fix in the next update I think.
