Update the length gonad score template headers

Measurement imports now include a reason field for mortality event measurements. Attempting to upload the length gonad score template produces an invalid headers error.
- Update the length_gonad_score_template file to include the reason header and empty reason fields
The public/samples/animal_count_measurements_template.csv file has an example of the reason header and fields.
