bookmarks and notes not being saved in questionnaire if not hitting next (or previous)

When in the questionnaire, toggling the bookmark or adding/changing notes don't save until the user hits next or previous. Changing the answer will also trigger these to save as well.

This will only affect users if they reload the page or close the questionnaire for some reason while on that question. It only affects the question currently being shown. This does not affect answers. Choosing/changing an answer triggers a save (and will save any notes/bookmark updates to that point as well).

Update the saving trigger to dispatch when a bookmark or note is also taken.
