Add function in new AE module to create dossier (combination of entered sheets and uploaded files)

To streamline things for reviewers, i.e. make a single, combined file for them to review on the site.

The CEC asked for a "cover sheet" as well, which includes the AE #, subject code, short description, and date of event - presumably as the first page of a dossier. It would not surprise me if they would also like to see basic demographics like age and gender on this as well. This information exists throughout Slice for any given subject (e.g. age/gender on Screening; date of AE on AE Report).
