Core rejects module_config retry sequence array sizes larger than 5

Expected Behavior

The policy server allows a user to create an arbitrary number of retry sequence numbers.

Observed Behavior

The policy server should not allow the user to create more than 5 retry sequences numbers. Remove the "plus" option once the limit is reached
