Description of problem:

So, we've got boss shuffle and Kholdstare is on top of Hera. The final two of Kholdstare's eyes were hit by the same fire rod shot and died at the same time, generating a lot of lag. The item is Progressive Glove #1, and then the crystal drops with a bad palette. Picking it up turned it into the Titan's Mitts and gave us the titan's, setting the compass item counter to 07/06. It acted as a normal item pickup rather than a crystal pickup, so we jumped down the pit and then climbed back up where the correct crystal was sitting there and acted as normal. The spoiler confirms that the second glove is on the yet unvisited King Zora.

Wasn't able to turn up any vanilla bugs like this, so thinking it has to do with the different arena.

Was anything else being used, such as a linked inventory co-op or ALTTP Online? Anything that could affect the state of the game?

No extra stuff, just vanilla randomizer. I wasn't directly the person this happened to and I'm having a harder time than hoped getting back to my friend, but that video is consistent with what I remember.
