Improve test coverage

Our test coverage has gone down, we should add some new tests to get back the coverage in the 95-100% range.
This is something that shouldn't be too hard for anyone to contribute to so tagging as "help wanted". https://coveralls.io/builds/3527804/source?filename=dist%2Fjose-testing.js shows which parts of the code are not covered by test cases.
