Component for `stencila-codechunk` nodes

Currently, Encoda hides the the code in CodeChunk: see code here. This was just intended to be temporary until we developed a component here that could be use to toggle the display of code.
One effect of this is that when a CodeChunk has no output it is not visible. As per stencila/encoda#145 (comment) CodeChunks should be styled so that they are always visible.
