Always creates 2 new folders

Whenever I right click and add folder, it always adds 2 folders.
If in Folder A I want to create a new folder called Folder B I right click and add folder...
I then rename the newly created folder to Folder B
If I then navigate away from Folder A and then back or simply refresh, Folder B remains but, every time there is another new folder appeared with the name New Folder

Not a massive issue, more an inconvenience
