I installed the module in a clean 3.5.28 Contao
I created a form with the Breacks page, but when I enter the form on the page I see only the navigation menu.

This is the code:

  • etichetta campo
  • Step 2

I've also tried in a Contao installation with a site already built.

It was not a bug.
You must also enter the form form, in addition to the extension module.
The instructions for using the extension are not very clear on this use.

What does that mean?

This means that the form module must also be added to the mp_forms module otherwise the fields do not appear.
I just thought it was just the mp_forms module

Sorry, I still don't understand what you mean. I would love to adjust the documentation if anything is unclear but I don't understand the problem you had.
