Support variable substitution in task mailouts

In order to make mailouts more appealing and personalised, it would be good
to support a standard set of variable substitution in the subject and body.
We already have {{ user }}, but we need a version of display_name that
always shows something usable in a subject line. (Or maybe "first_name" is
always fine for this).
It would be good to also support a {{ button }} option, so the "start"
"ignore" etc buttons can be placed wherever we like in the email.

Partially addressed in r393 , though not the subject text.

I'll close this -- the subject line's less important. We can always open a new
issue.
