Create a field name crosswalk for KIS team

The KIS team needs to be able to dictate the appropriate FIELD_NAME_USER values.

Create a simple .csv with two fields:

• FIELD_NAME_DEV
• FIELD_NAME_USER

KIS team members can edit the later field and Tiernan will update the development version of the dataset using a join.
