On fourth flash of controller lights, program crashes

What steps will reproduce the problem?
1. Run runSteelBatallion
2. Open Simple.cs
3. Click Start
What is the expected output? What do you see instead?
I should see five flashes and be able to use the controller. Instead, I see
four flashes with the fourth illumination remaining on constantly, until I
unplug the controller.
What version of the product are you using? On what operating system?
I am using 2.0.0 on Windows 7 Ultimate 64.
Please provide any additional information below.
I have carefully followed the installation instructions. Device shows installed
in device manager and working correctly. However, when looking at the test tab
for the controller, all axis show correctly for a second, then only Z shows
correctly. This is as far as I have succeeded in the process.

Repeating the process, I can get 1, 2, 3, and 4 flashed before crash. Also, I
am using the green light spring pedal version of the controller.

Update: I got passed the fifth flash one time. The program immediately crashed
afterwards. I have tried using different USB ports, and different .cs files.
All seem to exhibit the same variation of behavior.

Update: I tried this on a different computer running the same version of
windows. I got the same behavior. I have two controllers, and I tried with both
(both green button controllers). Same behavior with both controllers.

Problem ended up being traced back to a defective xbox->usb adapter.

Well, it is always simple once you know the answer. Over the weekend, I visited
my dad to install his controller and watch it fail. The only problem is that it
worked just fine! I was baffled. The only variable was the USB to xbox
connector he had was different than mine. He ordered extra connectors when he
bought his, so I took them with me. I tested them on my controllers this
morning. They work just fine! This whole time the problem was that the USB to
xbox connector I purchased (both of them) are pinned incorrectly.

Working connector: Ordered 2013 Length 3' Appearance: See attached file
Non-working connector: Ordered 2010 Length 6' Appearance: I don't have the
packaging any more. No branding of any kind on the cable. Doesn't even say
where it was made. Typical, all-black cable.
They are clearly different products.
