Improve upgrade instructions

- Update the documentation, and the comments in the demo.yml, to really describe what the ansible_host, connection and private keys are for. Give examples of both. Also, give examples for the api and user urls.
- Make special note NOT to bring down the docker containers when doing the upgrade
