We would like to make the following changes to the current routing slip. I am attaching an example of what we would like for the new one to look like.

I'm going to try to add a 'generated by' footer, if that's of interest, so you know who created the slip, and when. Right now it defaults to my user id.

routing slips have been retired in favor of the online workflow system
