Add phinx-example.yml to installer

We currently have phinx.yml frozen to defaults via git update-index --assume-unchanged phinx.yml in the installer. Maybe having phinx.yml in gitignore and move the defaults to phinx-example.yml might be more consistent with config-example.php.
