Improve document navigation and TOC

Based on this user feedback, we should investigate if there is some way to allow better navigation inside of the document on Kindle device. And maybe how to handle confluence generated TOC's pointing to the internet now.

This is a good idea but, unfortunately, the quality of the documentation is such that one
cannot fully take advantage of it. The TOCs, for example, don't allow you to navigate
inside the document via the GO TO kindle function, otherwise than to the very beginning
or to a given page number. The only way to navigate is via the links in TOCs but they are
links to web pages, not inside the document, and beside the fact that a wifi connection is
required, it takes ages to switch pages.
